This plan is available only to patients who do not have dental insurance. It is not an insurance policy, prepayment for future services, or a substitute for insurance under the Affordable Care Act.
Plan activates immediately upon enrollment and is valid for 12 months with automatic renewal unless canceled. Monthly payment options require on-time payments to maintain membership. Unpaid balances revert to full fee pricing after 90 days.
This plan cannot be combined with insurance, workers' compensation, or any other discounts or offers. Payment for additional treatments is due on the day of your appointment. Refunds are at the discretion of Viera Dental Design Studio.
